Judging from the screenshots, I think you should change the "Press up to read" sign, to something like a sign with lines/scribbles, or something. Just seems weird when it says "Press up to read", but actually says something else. You should probably have text saying "Press up to read" around the first sign to notify the player, but that's just me.

Only solution is to press 'R' or pass that phase before it happens. Probably a glitch since it sometimes happens and nothing seems to happen.

Defeating Bowser, Wart and Dr Wily several times and not realising I could backtrack to the previous save point.
Jumping into the Guy's mouth by accidently pressing the jump button.
Defeating Mecha Birdo, but falling to my death by the Shy Guy while Mecha Birdo falls. I know I could move on if I waited, but I didn't know at the time and pressed 'R'.

It's not the worst, but some of the collisions are messed up. I was almost fully inside a spike one time and then barely near one and "Game Over".

Dracula made me give up for a few months.
The Guy took me around a month, with a week's break during the war.

They're both quite easy now, though.  :P
