Stuck at the beginning

I just started playing, and I made it to the Game Over screen, which, as far as I can tell, is a dead end.  BUT, there is an item, a small blackish-bluish thing (I honestly can't describe what it looks like), that I get.

This items also seems to do nothing, then I'm stuck.

Any tips? hints? help?

P.S. I tried going back, but I can't get back to the right side of the cloud & spike screen

ride up the spike thing that drops

and that item was a secret item one of 6.

Btw, they do nothing as of now but they will eventually.
